	Greek Life Grade Release Form Description Fill Out This form is required to be on file with the Greek Life Advisor in Student Life. This will allow us to release non-public information to your chapter officials for national requirements. All potential new members must fill out this form prior to being extended a bid or invitation to join. Please note that organizations will submit a list of students they are going to be extending a bid to for reference. Volunteer Agency Questionnaire Description Fill Out Thank you for being a part of our volunteer database at Saginaw Valley State University during the 12-13 academic year. This database was created to help students find places to volunteer in the Great Lakes Bay Region. This survey is intended gather feedback from all agencies listed in order to better understand the usage and impact our students might be having on the local community.
